Our office staff will enter your complete patient information in our computer system prior to your first visit. A complete and thorough medical history is necessary in order to accurately evaluate a patient and recommend further medical treatment. This form should be completed when you come in for your first visit. The form can also be found below and filled out prior to arriving at our office. It is also helpful to provide us with pertinent medical records and x-ray on your first visit or have them sent to us in advance. If you wish to bring a family member with you, we welcome them as well.
